First Visit

Thank you for choosing CMG for all your family’s medical care needs. To assist us with your care, please register for our patient portal to obtain necessary paperwork for your first visit and fill out your Medical and Personal History information online for the physician. Please arrive at least 15 minutes early for your appointment and bring your insurance card and a photo ID. If you are bringing your child, please bring his/her immunization records as well.

Our physicians and medical assistants are experienced in the latest advancements in medicine. We believe that good medical care begins with open communication. We promise to speak candidly with you about any symptoms, our diagnosis, alternative treatments that may exist, and our recommendations. Our physicians are Medical Doctors (MDs) and Doctors of Osteopathy (DOs). Both types of physicians are fully trained and licensed. To learn more about their differences, please click the links below.

About Osteopathic Medicine

Family Practice
Family Practice doctors treat and provide comprehensive care for all ages and provide a range of services including preventative care, obstetrics, routine gynecological care, and pediatric services. They emphasize disease prevention and promote general health and well-being.

Internal Medicine
Internal Medicine physicians treat adults and specialize in dealing with complex, chronic illnesses. They focus on the prevention, diagnosis, and treatment of adult diseases. Internal Medicine providers are often called “internists” which should not be confused with Interns who are in their first year of residency. Internal Medicine providers practice in a variety of settings including private practice offices as Primary Care Physicians (PCPs), health care clinics, hospitals, and nursing homes.
